Do we still need to put our names and client id to those documents like air plane tickets, scanned copy of wedding details, etc

or do we only put names and client id to attached papers for the forms

We didn't. We just put a cover sheet identifying the contents of the envelope (on the cover sheet we did put our names, i.e. Sponsor: Fencesitter DOB: 04/17/2012, etc.).

They will have the application with the forms and proof of relationship together...
