What is your status? Are you a PR? If so you should have applied for spousal sponsorship a year ago.

Yes - you can request GCMS notes.

Unfortunately it's quite common for a spouse to be refused a TRV in these circumstances since IRCC knows your spouse has plans to remain in Canada long term (vs. just visiting).

Generally speaking, your spouse would want to show stronger proof of ties to the home country if they want to reapply and try to overcome the previous refusal (e.g. employment, property, assets).
