does the year awarded mean the year you received the convocation or certificate???

It should ideally mean the year you completed the course. Convocations could be later.
 
then what is the difference between year attended and year awarded? the application form shows both.

Year attended could be 2004 to 2007 (if a 3 year course) while year awarded would be 2007.
Convocation date and year would not really matter here.
