I am preparing my reference letter as a telecommunication manager and I have worked with 1 company for my past 15 years.

I've been in my last position for 5 years, so should I include the previous positions (which might be under a different NOC description) with the roles and responsibilities, or only the most recent one?

You only need reference for the position you listed under work history.
If you listed all NOCs and 15 years of experience under work history, yes you need reference for them all.
If you only listed last position in work history, then you don't need to include previous positions in your letter. Just add them in personal history section.

As per my understanding, any experience above 3 years dont have relevance. I gave only recent 3 years from my latest company for the experience letter proof.
