received a lette for remedical. Where to get the form from?

Hello everyone,

My wife received a request to get her medical done again. But there is no medical form in the packet. Can she use the form from Appendix C, the one which she used the first time for her medical or she has to contact CHC to get a different medical form?

This is URGENT.

Your wife's doctor will make her a file I think.

When I did my medicals I never had with me a form paper, I just had 4 passport size photo, that's it.

I don't think we need a paper form to bring to the doctor's office.

CHC-Islamabad provide the medical form with the letter. She overlooked it.
