I am a student in Canada and my spouse is on a work permit. Our visas are single entry. We need to go out of Canada for a few days. How do we apply for a re entry permit. Can anyone please give the link to the application form for the same.

You need to apply for a multiple entry visa

Generally students and workers are issued multiple entry visas. Check your TRV again it has to be multiple entry.
 
My Visa clearly states " does not authorize re-entry"
I already have a visa now, how do i change it to multiple entry?

There is a column on the TRV "Entries" and they enter S for single & M for multiple entries.

What does that column indicates on your TRV?
 
I do not have TRV (on passport). that is what i want!
I have a study permit on paper.
nothing like "entries" is mentioned on it

Educate me. How did you enter Canada if there are no TRVs on your passports?
 
If you are from a country which requires visa to enter Canada, you obviously would have gotten a trv of type S1 (being student) stamped on your passport.

Study or work permit is authorization to study or work in Canada, not to enter or exit it.

However, it is not needed for people belonging to countries which are visa exempt.
