Hi,

I have a query with regards to Additional Family Information Form that, do my self and my spouse need to fill the application in english and mother tongue. ? or just only in english ?

as at the top it's mentioned that you need to fill all names in english and native language.............. sooooooooooo?

Yes both of you need to fill this form and in both languages, English as well as native language. Type in English, print it out and write your names in native language with a pen.
