So let me give you people some context:

I applied for permanent residency under the spousal sponsorship program (inland)
This means for me that I currently reside in Quebec and will be staying here for the next few years.

I have an Open Work Permit valid for 2 years, case 27 without any restrictions.
I will receive my CSQ in about 7-14 days from now.

What documentation do I need to provide to apply for Quebec Health Care and how and where can I do this, once I have my CSQ?

Both Quebec and Ontario appear to be refusing health care coverage for OWP pilot participants. It seems you have to wait until you have AIP (i.e. first stage approval for inland).
 
I undertand that. They told me I need my CSQ before I could apply.
You will normally get the request to apply for CSQ at AIP.
However I am lucky enough to already be at this stage. I expect to get my CSQ in the next 14 days.

Now I need to know what other documentation I need to apply.
