I am sponsoring my U.S. wife and we were recently married here in Canada, and she is residing here with me now. We're almost done with the application but need some points clarified:

1. - Medical clearance. She had a medical completed by a Canadian Immigration approved physician in the U.S., who submitted the results electronically. How do we 'enclose' that with our application?

2. - IDENTITY AND CIVIL STATUS DOCUMENTS (#22) - I'm not really sure what's being asked here. My wife's birth certificate?

Also, applying outland gives you the appeal option, correct? Not the Mississauga location?

You should have received an electronic receipt after the medical (i believe via email) that confirms you did the test. We just printed that out for my husband and sent it off.
For the identity documents, you should read the guide for US applicants and it will give you a list of all acceptable docs - birth certificate, passport, etc.

You are applying outland, correct? Don't apply inland as an American unless you have extenuating circumstances. ALL outland applications, from anywhere in the world, are sent to Mississauga first by the sponsor/applicants, and then transferred to the visa office by CIC usually based on your citizenship, so yours will go to Ottawa after SA. Make sure you are on these documents, not the inland versions! Then, scroll down to "United States [IMM 3910] (PDF, 346.11 KB)".

Also, you should have received a document from the doctor's office. In our case we always got an IMM form with my husband's (American) picture on it. So, if you don't have anything, I suggest contacting the doctor's office.
