questions on pr renewal application

Kindly help on these questions I have to fill in the pr renewal application:
in 4.1 ---------- what date should I enter as last recent address, knowing that i still live at that address.
in 4.2------------ what date should I enter as last current job, knowing that I still work at that job.
in 5.5-------------- there is a box that says period to be assessed, what end date should I write.

last question for photo needed, where do you recommend i take these photos? and should it be certified at the back of the photo? any advice?

4.1. The date that you submit the application.

4.2. Same as above

5.5. It will be exactly 5 years from the date that you submit the application. Would be 26 Nov 2017 if you are applying today.

There are probably many places near you that do PR photos, so do a search. If I recall, the specifications are NOT the same as a passport photo, so be sure they know that you need PR photos. They will stamp the back with the information needed.
