Questions on filling the Application form for FSW- 2014

Hi Friends

I am filling the application form for 2014 -FSW.
I have few questions. Can you please clarify

1. In Generic application form(IMM0008ENU_2D), Q.no 4: Immigration office requested for procesing this application? I am currently on H1-B in USA. In CIC website, they mention CPC-OTTAWA as immigration office for people currently staying in USA and Canada. I am not sure whether Can i mention Dallas ,USA as immigration office since I am currenlty in USA?

2.In imm0008_3e. Schedule 3, economic classes -new federal skilled workers ,Question no.12: Occupation and NOC code: Do I need mention the occupation corresponding to the NOC code that am applying now or DO i need mention the job title currently i am working..

3.In Fee Payment Form,Section B-credit card authorization form, Can I use credit card issued by a US bank to pay the fees in CAD$.

4. In Schedule A,Back gorund decalaraion form, Q.NO: 12, Addresses: I was working in few foriegn countries about 10 years back deputed from my Indian based MNC IT company. since it was long time ago, I donot remember the complete address like street and number for the address i stayed in foriegn country 10 years back. so Can i just fillup address with out steet and number .

5. apart form this different question on points for relatives. do the relative needs to be close family relation or Can i use my any relative staying in canada

Please help on these questions so that I can fillup the application form.

I'm sorry no one has replied to you. I hope some senior who is more experienced than me gets back to you. However in the meanwhile, the following is what I think:

1. As far as the Visa office, you need to go here: http://www.cic.gc.ca/english/information/offices/apply-where.asp and select the visa office i.e. look at US under the column "Applications for Permanent Resident Visas" that would be CPC-O. To be eligible to apply from the US, you need your H1B visa to be valid at least 1 year into the future from your date of application. (Some people claim it should be one year in the past, but I am not sure about this. In the worst case, the CIO would send your application to the VO in your country.)

2. I am at work, so I am not sure what I put in here. I think I put in my job title, not the NOC title.

3. I know many people from India who paid with Indian credit cards, so US credit cards will do. Your credit card may charge a foreign transaction fee. The good thing about a credit card is that you get an indication when they charge it. However if for any reason your credit card gets cancelled, or the transaction gets declined, things get very interesting. (DD/Bank draft is the alternative, though most pay by CC)

4. You can always add an Addendum i.e. some text on a separate sheet of paper, explaining that you do not remember the addresses that you stayed at. I am sure they would accept this. Just put in what you know on the form, and attach the Addendum.

5. I personally understood it as close family member i.e. parents, siblings, and children. This: http://www.cic.gc.ca/english/information/applications/guides/EG7TOC.asp seems to say:

You or, if applicable, your accompanying spouse or common-law partner has a relative, i.e. parent, grandparent, child, grandchild, brother/sister, aunt/uncle or niece/nephew, who is 18 years of age or older as of the date we receive your application, and who is a Canadian citizen of permanent resident residing in Canada.

If you are getting 67 points without that, then it does not matter. If no, then please provide more details on how the relative is related to you, and may be someone might be able to help you.

I am almost certain that you are required to be consistent with your NOC code. You should reread the list of reasons for rejection of applications for FSW programme.
