Hello,

I have a couple of questions about The additional family information form (IMM5406).

I am Canadian citizen sponsoring my Greek husband. He is the only person whom I'm sponsoring as no one in his family is interested or looking to come to Canada. And he has no children.

Is this form relevant? Do we still need to fill it out and list his parents and siblings on it?

Yes, his family still needs to be listed on the form.

This form will form (partial) basis for the immigration file being created. They need to know about the PA's family. Only people listed on the Generic Application will be assessed for a PR. Other family members are always examined, as part of the process.
 
Does that also mean we need to also provide birth certificates and national identity cards for his family as well?

Also would we need to provide police reference checks for them too?

Or is the ID and police only for my husband whom I'm sponsoring for PR status?

No, just the details on this form is enough. Keep in mind, you have to fill it out twice (once in English, and once in Greek). They can arrange the required checks, as they need.

Only your husband will need to supply ID and police checks, etc.

no, his family members don't need to provide all that information because they are not applying for PR. they ONLY need to be LISTED on your husband's additional family info document. this is what IRCC uses to identify possible future sponsorships. his family that is not applying for pr do not need to fill out any forms or provide any personal documents.
