question regarding sponsorship application form

hello folks,

I am preparing the sponsorship application form. my question is who is suppose to fill the schedule A background/declaration form Imm-5669.

I am sponsoring my wife. are we both suppose to complete this form ? or is it only my wife who needs to complete the schedule A background declaration form?

My another question is in regards to sponsor questionaire form imm 5540. on line 11. it does ask are u currently living in canada? since when? your address in canada.
I lived in canada for 2 weeks during december 2011 and I am currently out of canada ( temporarily) going back on 1st week of May. now should I put the december 2011 date on this form or i should put the new date when i return back to canada?

Your wife is to fill it out, it's about her background.

I'm not ENTIRELY sure about the second question, someone else might be able to provide a better answer, but I believe if you are not living in Canada right now, you check NO. UNLESS you are planning on applying after you move back to Canada, in which case you would put the address you'd be staying at. Tricky question, like I said that's just from what I understand but a senior member might be able to be more helpful!
