Question about the declaration of common-law and supporting documents

So I'm submitting a notarized declaration of common law, as it seems to be required in my OWP application. On the declaration itself, you have to choose which evidence you have (i.e. lease, property, life insurance). Would I have to submit these documents (in my case a lease) within my OWP package with the declaration?

Also, will I have to bring them to the notary?
