question about online questionnaire to extend TRV

On the online questionnaire for extending your stay as a visitor it asks:

are you accompanying a family member who has status in Canada?

What should my husband put. I'm a Canadian citizen but I am already in Canada, and he is also already in Canada extending his stay.

Also, when filling out the form online it says "date and place of original entry into canada"
does this mean the very first time he came to canada (on a tourist visa that is now expired) or does it mean the first time he came to canada on this visa that is still valid?
