So it says I can't do the exam until I get instructions to do so, but should I try to make an appointment ahead of time or do the physicians give priority to people immigrating? I just want to make sure I'm able to get in.

You can of course ONLY have a medical with an examiner approved by CIC and on their panel. Such Doctors normally do this as their main source of income and can do it within the timescale granted by CIC, so no need to stress about it or 'book' anything until you are asked.
