Question about Form Additional Family Information [IMM5406]

Hi,

I have question regarding an email I have received from London CIC office. Please see this email as follows,

Dear Applicant,

We are writing with regards to your family-class application for permanent residence in Canada.

Please be advised that per our email of 08 February 2017, in order for us to continue processing your application, we require the following for yourself and your spouse:

- Medical examination results.

- Please have your sponsor complete a new Additional Family Information [IMM5406] form.

Please submit the requested documentation/information to the London visa office within 30 days including a copy of this email as a cover letter. Please submit all the above documents to us at the same time, quoting your file number (shown in the subject line above) on all correspondence.


--------My question regarding this email-----
Question1:we already had medical examination and we are in impression that affiliated examination centers directly sends medical results to London office. Is that true or we need to send them some thing as medical exam confirmation?

Question2:I see IMM5406 is for principle applicant but they are asking sponsor to fill this form. Please advise

when doing the medicals, they send you home with a form that should have a barcode. that was supposed to be sent in with the application -- depending on where you got them done, they may have already expired so it looks like a new one is needed

as far as the Additional Family information, i am not sure -- i would suggest having your sponsor call CIC and clarify on that one
