Question about Family Information Form - Schedule 1 (STUDY PERMIT)

Hi everyone,

I have been offered admission at the university of waterloo and now I am filling out my visa application for study permit. I am a bit confused in the Family Information Form -Schedule 1 where I have to fill out my job details for past 10 years. OK, I have filled it for past 4 years as I have experience working for Motorola and Nokia Siemens Networks, but I am confused for the 6 years prior to that. I was dependent on my father so I have to fill out my father's information obviously. Here is the confusion... my father is a Self-Employed person and runs a Medical Store. That is and was his source of income. Now what shall i fill in the form, shall i fill my Father's Name of His Shop's name in the Employer details and shall I fill my father's house address or his shop's address in the form.

Experts... I need your valued suggestions please.

Refer to Guidelines 5296E. Page 19 & 20. The guidelines (though are for IMM1294E) mention that if you do not work, you can write about your activity e-g: Going to school etc. In order to cover the previous years you should write "Studying - (Undergraduate - Engineerin), Profession: (Student) Dates: (Duration of your Degree).

The previous version of visa application forms (older) had Education History / Travel History as a separate part and one had to mention this for the past 10 years.

Makes sense for Schedule-1, however what about 'Supplementary_Information_Form-EN' a.k.a 'Supplementary Information Form' a.k.a 'Family Composition Form' which CHC in Islamabad asks us to fill. It's mentioned in the form if you do not earn and are dependent on someone like husband or father, than fill that person's details.

You dont need the Supplemetary Information Form (that hand written one) with your visa application. Instead you should be attaching the Family Composition One IMM5645E. Simply write the details mentioned and tick if anyone is accompanying you.

PS: Im saying this because I had the filled Supplementary Information Form with me, but the submission officer said its not needed and handed it back.

Strange ... it's the very first requirement mentioned by CHC Pakistan on their website:

"The following items are required for a complete application:

- The application form must be completely and accurately filled out, including the Supplementary Information Form PDF * (38 KB) (also referred to as the Family Composition form)."

Ok, need one more suggestion. I have been on the same job for the past 4 years and during this time my job title has been the same but salary has been incremented thrice. So do I need to fill in this complete tenure as one single entry dating from Dec-2007 to Nov-2011 with my latest salary or shall I enter each incremented phase as a separate entry?

Yes that is why I had the form with me at the time of submission. But they handed it back.

2. Since there isn't any change in your designation, you don't need to fill up your employment history according to your increment. Just write it as a single entry with a 4 year span mentioning From - To. As per the Salary, write down the latest since you might attach your salary slip (latest) with your application.
