Question about "Additional Family Information" IMM 5406E

It asks for your "present address".
Would this be my address here in Canada where I have been residing for over a year?
Or would it be my actual permanent address in the US?

Also, it asks for my father's current address but I am not aware of where he lives as my parents are divorced and we have not seen each other in 7 years.

For your father's address write "unknown" or "last known address is...". If you can provide city, state, or country with reasonable certainty then list them. Don't just leave it blank.
