Question about Additional Family Information [IMM 5406] (PDF, NaN undefined)form

I am in the process of filing spousal PR application for my wife in India. My situation is simple. I live and work in Canada. I just married her last month, and she does not have any children.

On the CIC link mentioned here below,

does the form "Additional Family Information [IMM 5406] (PDF, NaN undefined) " need to be filled by BOTH my wife and a separate one by me as well? The guide here http://www.cic.gc.ca/english/information/applications/guides/3999Etoc.asp says this form must be completed by the principal applicant, and Your spouse or common-law partner (whether accompanying you to Canada or not).

So, I am a bit confused. Do I need to fill this form "Additional Family Information [IMM 5406] (PDF, NaN undefined) " too? I don't think so, but want to know your inputs, friends. Also, does my wife need to mention my name under the Spouse on this form?

She would fill that form out. The checklist is very helpful on which forms are needed and which is for the sponsor and which is for the applicant.

No, you don't complete the IMM5406 Additional Family form, only your wife does. Yes, she needs to add you to the form as a Spouse.

Like bradybunchmom mentioned, follow the specific checklists and that will help you soo much. You will only complete the forms on each checklist.

Some of the forms are generic and so in case there was a couple who wanted to apply for Permanent Residence on their own (No sponsor) and neither one was a Canadian citizen, then they would both complete that IMM5406 form.
It can be confusing, but just follow the checklists and we are always here to help.

I'm in the same situation, but just to be sure, my wife needs to write down the names and adresses DOBs of her husband sponsor living in Canada (me), her parents (who live in the Philippines), and her brothers and sisters, even if they are not part of the sponsorship process?

And since they are not included in the sponsorship, they do not need to fill a copy of this form themselves?

Right. Your wife fills it out, and includes you as her spouse, her parents, and all siblings. You do not fill out a form, and her parents and siblings also do not fill out their own forms.
