Query regarding POF

Regarding POF, my wife has an old Fixed Deposit which we plan to use as POF. She need to give a letter stating that I can use the funds for settling in Canada.

Does someone has a sample letter for this ?? I am not sure whom to address the letter and how to state it (As a third person or second person).
