Query regarding including Salary Per Annum and Mid Year Appraisals

Hi
I have a query regarding including per year salary details in the reference letters
my scenario as as follows in my current company :

1st April 2010 to 31st March 2011: Rs. 1,12,000/- (per year)
1st April 2012 to 31st March 2013: Rs. 2,24,000/- (per year)
1st April 2013 to 31st March 2014: Rs. 2,50,000/- (per year)
1st April 2014 to 31st March 2015: Rs. 3,20,000/- (per year)

How do I ask the company to put in the following year where I had a mid year appraisal in January

1st April 2015 to 31st December 2015: Rs. 3,60,000/- (per year) ?
1st January 2016 to 31st March 2016: Rs. 4,04,000/-?

or do I calculate it per year by adding in my previous salary and my appraisal salary ?

Just get the last drawn salary at each position!
