Query on Child's Birth Certificate

Hi ,

My husband is applying for PR under the CEC . I have some queries related to the Birth Certificate of my child. I have always used my Maiden name which appears in my Passport and other documents.

But the Child birth certifiate bears my Husband's Family Name.



Please let me which could be the best option for me :-

1. Do I have to mention in the PR form ( as I have never used my husband's family name ) it was by mistake the birth certificate of the child shows my husband's family name.


2. Do I have to give a self declaration of the above facts.

3. Do i have to submit the affidavit duly notaritized about my declaration.

or all the above.

If I understand this correctly, you still have your maiden name on your passport and in your child's birth certificate your full name does not have maiden name but has your husband's family name instead?

if so, you need an affidavit to explain.

I have never used my husband's family name anywhere . I have always being using my maiden name post marriage.

by mistake the child's birth certificate states my husband's family name as my family name .

what should I state as my diclaration in the affidavit . I mean the legal language.....


in simple words all i have to mention is that I have be know by my maiden name and my daughters birth certificate bears my surname as my husband's family name which I have never used.

Can someone help me with this ... Unable to find any such sample format

Go visit a lawyer, he/she will have the format and you get the affidavit quickly. You may expect 50-75$ depending on the lawyer.

or, depending on where it was issued, you could save yourself a lot of headache by applying for a corrected birth certificate.
