Queries related to Form IMM5444 for PR renewal card

Hi Everyone,

Could you please help me understand how should I fill the details for these queries on Form IMM5444 for PR renewal card:

3.5 – Travel documents​

Check the box(es) only if you have ever been issued one or both of the following documents:

• Travel document
• Permanent Resident Travel Document (PRTD)

I have not been issued a PRTD before, I only travelled before using my passport and PR card before. Do I have to select Travel Document checkbox and provide my passport details here?

4.1 – Address history​

I have at 3 different addresses within Canada since I became a PR. I will add this detail (from current to old) but do I have to mention addresses of the places I have stayed at while travelling outside of Canada ? I have mostly travelled to meet relatives or my daughter outside country and have stayed there for few months for couple of times.
