for a contract job I did

Employer Address: Toronto
Physical work location: Windsor

Address in ROE is in Atlanta USA as the contract house payroll department is in the USA.

So which out of the 3 should I put in the address field?

I also agree with CaBeaver... Location should be Windor
In my case HO of organization is in Oakville, but my office is in Vaughan.
So I mentioned Vaughan in application form
