The Canadian Embassy in Manila has sent my husband a letter asking him to provide a signed declaration stating that he understands the consequences of not having a family member examined for admission to Canada. This declaration should be attached to the email but it wasn't. Has anyone experienced this? If so, what declaration form are they talking about? My husband has emailed them already, but was trying to see if he could get a head start in preparing this document instead of waiting around for them to email him back.

Here is the most important message of the letter:
"Should you still wish to proceed without having this family member examined, you are required to submit a signed declaration stating:
(i) That you have been counselled by an officer of the consequences of not having your family member examined
(ii) That you have been specifically made aware of the provisions of paragraph 117(9)(d) of the Immigration and Refugee Protection Regulations; and
(iii) That you understand that you will not be able to sponsor the application of this family member as a member of the family class at any time in the future.
Please carefully review the attached declaration."
