Proof of Medical? Online forums not loading.

So, last night I went to apply online for my visitor extension while waiting for my permanent residency. Online, they want a proof of medical, but I don't have anything. The only thing I received from the doctor was a receipt that I paid for both appointments and what date they were on and that they were for an immigration medical. The doctor said I would get one as soon as he submitted the paperwork, but he couldn't do it right away because I had to get something faxed to him before he could send it out. Anyone know if the receipts are good enough, or do I need the actual forum from the doctor himself?
Also, is anyone else having problems getting the forums online to load? I tried multiple times, on multiple computers on multiple programs (Comodo Dragon, Microsoft excel) but the visitor extension forum will not show up online for me to download and actually fill out. This was last night so I'm going to try again in a few, but if anyone else had that problem and knows how to work around it could anyone let me know how.

Well I finally got the extension forum to load, but now my question still stands with the proof of medical, can anyone help me out with that question?

Was your medical done by a panel physician? I got a print out with information about the medical to be submitted with the visa.

Yes, it was done by a doctor that was on the list. I never got a print out, the only thing I have is the email with the instructions and the forum you take with you to the doctor. I have nothing from the doctor himself or the clinic..

Did the letter that you received from CIC have a bar code on it?

Call the doctor's office and ask if they use the e-Medical system, which transmits the medical directly to CIC.

Yes, it had a barcode on it. They use the e-Medical system, but the problem is I need the forum that is the proof of the medical. I called but they're closed so I just left a message, but I was wondering if there was anything else I could submit instead of the proof of the medical worse case scenario, or if I HAVE to submit the proof.

That IS the proof of medical. CIC just needs the barcode.

I see! Everything I saw was saying it had to be a forum that the doctor signed saying I had the medical done on this date at this time and it was sent out on this date and this time.
