"Proof of medical examination" on IMM 3910

Hey guys, just a question.

Husband just did his medical exam today, we're getting ready to mail out the application. But I only have the information sheet, not the receipt. We were not given a receipt.

Is CIC expecting an information sheet + receipt?

They should have given you a receipt for your xray and blood work and the exam Your name and address and a number is usually attached to it .

All I sent in was a copy of the information sheet, they gave me 2 copies, told me to send in one and keep the other for my records.

Yep and it had a barcode up top with a UMI number.

That's what i sent plus blood work and xray receipt . When you said information sheet our Dr gave us one with some facts on medical for immigration .

I'll just take a chance and include only the information sheet... It can't be that big a deal.
