Proof of Employment Format

Kindly guide if there is any particular format for the Proof of Employment letter that I need to get from my Employer?

Any specific address that should be mentioned in my PoE letter?

TO WHOM IT MAY CONCERN:

I am writing this reference at the request of ABC who is applying for Permanent Resident status of Canada.

In his position as Network Specialist, ABC is employed in our organization XYZ since December 2007. As a full-time employee, he is mainly responsible for implementation, migration, testing, troubleshooting of ISP customer networks. His duties include working with different networking protocols, providing network security, network quality of services, installing networking hardware and software, monitoring and analyzing network traffic, and solving advanced networking issues. He also provides network related support to the ISP clients. As Network Specialist, he is working 40 hours per week and currently earning $XX,000 per year. Besides salary, he is also receiving other benefits such as 2 weeks of paid vacation per year, extended health plan, dental plan etc.

And if employer is not willing to give employment proof in above format and has a specific one?

I believe there is no specific format except you should able to give paperwork which shows that you are currently working as paid worker and fulltime worker with your role & responsibility.
You can give multiple papers..like you can show your annual income statements/tax statement or payslips which will show you are working as paid employee.
Frankly speaking, I gave almost all the papers which I can give to support my application.
