You can fill out the forms again, but CIC will be expecting them to be identical to the forms submitted at the initial stage, so you should include a coer letter explaining the situation and why they may be different.

As for your fee receipt, if you have the file number, you should be able to confirm your application status online, google CIC e-CAS.

Since your former representative could "withdraw" your application and (and receive the fee refund), it's imperative that you submit your "withdraw use of representative" form ASAP...
