problem in filling the "family information" form

i have a problem in filling the form about my "family information".
my father has deceased. items in "marital status" for family are:
*Annulled marriage
*Common-law
*Legally Separated
*Married-Physically present
*Married-not Physically present
*Single
*Widowed
which item shall be selected for my father?
