Your medicals are done so your are sage, just do your landing as soon as you receive your passport and have your baby delivered in Canada.

Yes, assuming she is not so far along in her pregnancy. Airlines have rules about "how pregnant" you can be to fly. You will also want to check the health insurance situation of whatever province you plan to settle in. Many have waiting periods before you become eligible for health insurance.

Yes she can because she will be a permanent resident and the baby will be a canadian citizen automatically. If she deliver baby back home then you will need to notify the visa office and it will take a bit more time. So better to have a baby delivered in Canada will be much easier in terms of paper work.

You are the spouse so your name will be used as a father name. You guys are now living together so your name will be used. But its my thinking i am not excatly sure about this situation. I would suggest you to take advise from seniors like Qorax,Maaties, PMM.
