PR Card expired, urgent renewal

Hi Experts

My PR Card got expired and need to renew it urgently as parent not well back home.

Any first hand info would be helpful on my questions below:
Would a Doctor Letter be just sufficient? (As per my call with CIC customer service)
Someone told I need "confirmed tickets" too!
