PR Application returned

Hi All,
My wife's PR application was returned today, and the only error was her birth certificate. However, the error message accompanying that was 'This document is incomplete, please check and update'
Has anybody faced this issue? Can someone please let me know what the issue might possibly be?

The message is most likely generic.
Is it an official birth certificate? Was it properly translated by a certified/official translator? Did you include both original and the translation in the same PDF? Is the scan of a good quality?
