I have a query regarding a question asked in the personal details section. The question there is, 'has this person ever used any other name?'
Suppose, in my passport I have Mohammed and in the other documents it's Md, will that be considered as other name??
The other query is regarding a question in the personal history section, where it's written, Have you listed all of your period of military service or government positions of any, and all of the organisations that you belong to?
What should I select over here??
Kindly reply asap as I have to submit it!!

Affidavit has to be made?? Md is just an abbreviation of Mohammed.

Your Visa Officer/ Case Officer may not know that. DO you want to take a chance basis their awareness? I know many in people in US who would not be aware of this. SO can be the case in Canada. Its better to submit affidavit
