Post graduation work permit

Hello,

I have completed my program last month and am going to apply for post graduation work permit.
I am little bit confused about the documents needed.
Does anybody know if I need IMM5645 which is family information?
I thought this form is for people applying from outside of Canada, but when I tried to apply from online, this form came up as a requirement.
Does anyone have same experience?

Ammm I don't know exactly about the forms but from CIC website you can download the 'bundle' to apply for PGWP. The documents contain in that bundle will be mandatory I guess.
