Police clearance certificate

Hi All,

I want to apply for police clearance certificate from Delhi but there are two fields which are confusing to me. Kindly help me here.

Note: I want pcc for spousal visa

1. What details to give in embassy details? - embassy name, embassy address, issue date, expiry date?
2. Attach letter of agency requiring pcc - I dont have any letter as I will be needing this pcc for spousal visa.
