Police Clearance Certificate & proof of fund

Do I have to send the original certificates or copy? I m sure it must be the original but just want to confirm it.

I have account with Chase, so regular statement will work as proof of fund or does it has to be in specific format? If it has to be in specific format then can someone share format or how to request it?
