Police Character Certificate Form - Need some help!

Hi,

I may need some help filling up the Police Character Certificate Form for Pakistan provided on the consulate general website. It looks pretty simple but a bit confusing at the same time. Will appreciate if anyone could help me to fill in the following form fields.

#13) Visa No. (Should I fill it with UCI number or TRV document number ?)
#18) Residential Permit No. (Should it be filled with Temporary Resident Permit document number something like Uxxxxxxxxx?)
#20) Last Extension Granted (Should it be filled with the issue date of Temporary Resident Permit document )
