PNP Stages -- I am so confused!!

Hello guys,
Can somebody tell me what the different stages are (in order).

1- Apply for Nomination, and issues nomination certificate
2- Send application to CIO
3- CIO forwards the application to Visa office
4- Visa office contacts for Medical examination


It will be great if you can also tell me what are the application status that appear on the ecas at every stage...

for step 1 there is no update in ecas, after step 2 each step there is an update on ecas

Is it possible that ecas is not updated? I mean, I received the request for medical, but the system does not show it. It still says Application Received, which (according to their table) means that my documents are complete, but still at CIO
