Please help!

Reading on a different post here I came across a concern. When we paid the fees online apparently we were suppose to fill out a form to send with our application. Which form was this as when we paid all we had was the receipt and that is it. We did not fill out a separate form and send it with our application!! Is this true that this needed to be done??? If so what should I do??? I would hate for our application to be sent back to us....Thanks in advance

Including the receipt is enough.

Yeah I had the same. My form had nothing to fill out. Was weird. I just sent it and hoped haha.
