Please help : why would IRCC ask to resubmit new medical when first medical been extended ??

Hi,
I am a PR applicant and I work in Canada under a WP for 2 years now. I have submitted my first medical on Mai 2019. Security check never been initiated yet...
Last Mai the IRCC decided to extend my medical because of this covid situation. but today I got a request to do a new medical check !! like seriously what's wrong with those agents ??

They might have extended the medicals for 6 months and now might have requested the new medicals.

they haven't mentioned anything about 6 moths before. the weird thing is this agent had the time to request a new medical but never bothered taking a look at my file and initiating the security check since Nov 2019.
even when I spoke with our PM I was told my medics will be extended as long as I am still living in Canada

How do you know that the security check has not yet been initiated?
