Hi i am collecting documents to sponsor my wife. where i can find the form IMM1017 copy of form for medical requirement and is it necessary to submit medical at the time of initial application or CIC will ask at later time once you submit your file to Mississauga?
 
The Dr will provide you with the IMM1017 when you have completed your application. Are you applying Inland or Outland? If you are applying Outland you will need to include this form as proof your medical has been done. If you are applying Inland I have heard of people waiting until CIC requests the medical.

I think he is applying outside since he's mentioned Missisauga.

Yes, I see that now.. So the OP will need to send proof of Medical IMM1017 with the application.
