Hi there. I had a question about the Canadian study permit application. Once you have successfully submitted the application for a study permit they make you sign another form. The form consists some space for signature and date and three no/yes (if 'yes' explain) questions. I wanted to know what are the three questions. I believe two of them are something regarding discrimination and arrest/crime record. what was the 3rd one? thank you.

You are definately talking of form IMM 1294.

This is the main application form and it contains 4 pages.The question you are asking appears in the last page which is page 4.
The qstns that contains YES/NO asnwers are actually 5 on page 4.The last 3 are?

- Have you been employed by govt in security related capacity?
-Have you ever held a position of authority in any govt,judiciary or political party?

Then you validate and a new page emerges with some unigque codes and signature line.

So what exactly did you want? Am waiting to respond.

No. not the application form. it was a one page form given to me when the application submission was being finalized. Edit: It is not done online, it was in CHC, after submitting the documents.

the problem is following:

I applied for a visa on June 14th that got refused on 23rd. Reapplied again today. The first time, after submitting all the documents they gave me another form to read and fill up. That form had some statements under which I had sign and put date. And there were three questions, for example 'have you ever been arrested or charged for criminal activities?' (something similar) with the answering option 'NO/Yes (if yes, explain). There were three, one about this arrest thing, one about racism, sexism and stuff like that but the other one I don't know about. At the time of submission I remembered how last time i read them and checked 'no' for all of them so did the same this time without reading it (i know i'm stupid but let's move on ). Now my worry is whether that last question was regarding 'past visa refusal'.

All of them applies to both cases whether fresh applicant or re-applying.

You realise the qstns are captured in both situations.

I am guessing you applied at VFS. Are you from Bangladesh?

About the questions, even I cannot remember them but all of them are, most probably, for criminal records.

Yes I am from Bangladesh. Applied at VFS the first time, this time went to HC. At VFS they gave two forms, this one and another one to write down the address and stuff. HC gave only this one. In my head i thought i checked all of them 'no' before so i can do that again. The moment i stepped out of HC it hit me that i am a 2nd time applicant and the answers might not be the same for me anymore.

Don't worry that much. I am pretty sure it was not related to anything about 2nd time applicant. BTW which university/program/degree are you going for?
