the form says BEFORE YOU START, READ THE INSTRUCTION GUIDE TYPE
but where can I find that Instruction guide type? if you know the link please give me

Question 2 B) .. it asks for the name of the supervisor, can I put the name of the human resource manager of my company?

I'm not sure if this is correct. Some people said Net Personal Income is not required if I only sponsor my wife, but I have to provide the Option C printout of the last Notice of Assessment

then do I have to complete the question 3?

The guide depends on what country you are applying from. So, what country?

You can put the HR down yes. And as for the income, I put it down and included Option C. They DO need paystubs or something just to show that you can and will support a family (yourself and your spouse).

Yes your immediate supervisor or owner of the company

how much you make every two weeks(every pay) X 26 (how many weeks you get paid)

the amount after tax .. or after all deductions like Union, Fed tax, CPP and benefit?

You should have a line that says "Net Pay" on your paystub
