Please help - Affidavits

Hi all, i'm a bit confused.

On the country specific instructions of my country (Israel), the checklist in no.2 "IDENTITY AND CIVIL STATUS DOCUMENTS" mentions to provide:
"affidavits attesting to the fact that you have been living together
and representing yourself to your community as living in a conjugal
relationship for at least one year"

for common law relationships. what does it mean? should we go and write it down before a notary public? what is the different betwwen this and the statury declarations we ask from family & friends ?
we proof our relationship with joint lease and like to supply as much ducoments as we can...

any help will be great ( :

Use this form and take it to a notary to swear it.

Instead of Conjugal I would just read it as "Marriage-Like."

Thanks, but i'm still confused.
The form you suggest to use "IMM 5409" need to be used when the sponsor have a co-signer (according to the sposnor checklist). Since i do not have a co-signer can we still use it as the affidavits I asked about?

I'm afraid they will look at that as if I made a mistake and have a co-signer (though I can't because i'm sponsoring my common law).

what do you think?

IMM 5409: it is true the instructions state that this is for when you have a cosigner who is your common-law partner, so if you are sponsoring your common-law partner you don't need it.

However, many visa officers want this form anyway for common-law applicants. Some people on this forum have reported that when they did not include this form, the visa officer processing their application then asked for it later. Sending it in even though it is not required according to the instructions will not hurt your case. At worst, the VO will just ignore it. But you might get a VO who wants it, so sending it with the application at the beginning will save time later.

So my suggestion is to fill in the form, get it notarized, and include it with your application.

IMM5409 is not applicable for spousal sponsorships, correct? We did not send this form because it was for common-law union, but we included it on our appendix to the document checklist stating it is not applicable to our situation (along with other forms not applicable - IMM5476 and IMM0008DEP...etc.).

Thank you for this canadianwoman.We went to the commissioner of oaths today to suppose get notarized the statutory declaration of severance of common-law relationship of my partner yet this guy told us that we need that statutory declaration of common-law bcoz according to them we need it.I told him it wasn't in the required documents nor stated in the document checklist but oh well to make the story short we did have declared the common-law thing but when we got home i found out that he missed to notarize the severance declarition aside from that instead of stating that we have the lease agreement he wrote there mortgate agreement. so we need to go back there next weekend to change this.
