Placeholder upload option for forms

I need to add my new born children to my application.got 2 Upload placeholders in my application.Am really confused with this.How do i need to upload the IMM0008 ,IMM5406e, birth certificate,passport copy etc for each child?Do i need to use one upload placeholder for each child?
Please help
