You will have to research the various immigration programs included in which will be the need to pass a medical with an authorised doctor. So if you qualify for immigration , pass a medical and have a good level of English then sure you can migrate to Canada.
Not sure what you mean can you apply to any hospital, for what purpose exactly given hospitals have no influence on immigration matters and as a foreign national you would not have any provincial health coverage.
As said above, you will need to qualify for permanent residency just like anyone else based on your level of education, skilled work experience, language abilities, age, the money you have saved to pay to move and settle yourself in Canada and other factors.
There is no special stream for those with disabilities.
