PGWP- family information

Hi everyone,
I am just graduated from Seneca college in Toronto n I am about to apply my PGWP.
There is a supporting doc. called family information. I dun know what is that and I am not applying any permit for my family.
Can anyone please tell me If that is normal and Am I must complete the form?
Because my friends does not have that when they apply for their PGWP two weeks ago.
Also, am I must stay in Canada until the permit has been approved?
P.S I am holding a HK passport so I do not need to apply visa to enter Canada.

I believe this is the only form? -> Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker (IMM 5710)

You have to apply for PGWP, before your study permit expires.
