PERSONAL NET WORTH STATEMENT... need some help

I have enough settlement funds, right enough to satisfy the required amount.
i deposited in two bank accounts, the first half is in my CLP's bank account...
while the rest is in our joint account.

question...

A. BANK DEPOSITS - CLP's bank account
B. PROPERTY - N/A
C. PUBLICLY TRADED STOCKS - N/A
D. BUSINESS - N/A
E. PENSION, MUTUAL FUNDS AND OTHER ASSETS - joint account
F. MORTGAGES - N/A
G. PERSONAL DEBTS - N/A

or will I write N/A in the mutual funds part and type in the two accounts at A. BANK DEPOSITS...

another thing...
on the part that says "WHICH IS DISTRIBUTED AS FOLLOWS:"
funds to accompany me to canada - 10,000
while the funds to transfer to canada, is the remaining amount in the bank?
is that how we are suppose to indicate it?

i'll be submitting full documents tomorrow, and i hope someone here can help me fill this out? im so confused...

is the mutual funds considered the JOINT ACCOUNT?

HI,

I'm a Pinay....if you have joint account just ask your bank for the certificate, about bank statement just submit past 6 months bank transaction..if it is in Manila just ask certificate to your bank, certify a c opy of your bankbook, then as your cover letter indicate per # what kind of account it is and how you acquire it.

im asking how to fill-out the form.
where to categorize the JOINT ACCOUNT?
is it in the BANK DEPOSIT OR MUTUAL FUND?
